Addl Notes: Observed no vomiting/diarrheal procedure or clean up kit at the time of the inspection. Written procedures for clean-up of vomiting and diarrheal events is required onsite at all times. Provided person in charge guidance/procedures for cleaning these events at the time of the inspection.

Addl Notes: Observed no paper towels at the bar handwashing sink near the coffee machine/smoothie area. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided hand drying provision for handwash sink at time of inspection.

Addl Notes: Observed raw skirt steak above soda and pancake mix powder in the walk-in cooler at the time of the inspection. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code.

Addl Notes: Observed dicer and chopper stored as clean with food debris still intact on equipment. All food-contact surfaces must be cleaned to sight and touch. Items were placed to be re-washed in the dish at the time of the inspection.

Addl Notes: Observed no chlorine sanitizer in the bar dishwasher in use at the time of the inspection. The sanitizer bottle was seen to be empty at the time of the inspection. Bottle was replaced and primed to provide the correct concentration of sanitizer at the time of the inspection.

Addl Notes: Observed hash browns sitting out on the cook line at 92'F, red salsa sitting atop ice at 59'F and ramakins of whipped butter and stock buckets (labeled keep refrigerated) at room temperature at the time of the inspection. Hash browns were placed out of refrigeration about an hour ago and was moved to a cooler at the time of the inspection. Red salsa and whipped butter were discarded as they had been out for greater than 4 hours. Spoke to person in charge about creating a time as a control policy for these items. REPEAT VIOLATION

7-102.11 - Priority Foundation: Common Name-Working Containers Addl Notes: Observed two generic spray bottles labeled without the common name. Person in charge discarded one they believed to be bleach and the other was labeled as fabuloso at the time of the inspection. Ensure common names are labeled on generic chemical spray bottles.

4-302.12 - Priority Foundation: Food Temperature Measuring Devices Addl Notes: Observed no food thermometer present in the establishment. Predefined Comment: Properly calibrated probe thermometers are required at all times for monitoring food temperatures. Must provide a probe thermometer for establishment prior to re-inspection (Friday, June 27th).

Comments:
6-202.13 - Core: Insect Control Devices, Design and Installation Addl Notes: Observed open fruit fly traps around the kitchen and bar at the time of the inspection. All insect control devices should be enlosed and away from direct food areas to prevent contamination. Traps were discarded at the time of the inspection.
6-501.111 (A-B, D) - Core: Controlling Pests; Monitoring Addl Notes: Observed rodent droppings along the back of cooking area in prep, dry storage, dishpit area and in the bar at the time of the inspection. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. REPEAT VIOLATION.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
