Addl Notes: Observed manager on duty handle raw breaded chicken for a customer order, place in fryer, remove gloves, dawn new gloves, and contact steamed cabbage directly for a customer to-go order all without washing hands. Corrective Action: Corrected at time of inspection. Predefined Comment: Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Predefined Comment: Employee properly washed hands at handwash sink at time of inspection. Predefined Comment: Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination.

Addl Notes: Observed female staff member cooking chicken from raw on the flat top. Chicken was placed in a to-go box to be sent out for a customer order. Inspector took a cook temperature, 148*f degrees in multiple spots. *Violation* Chicken was immediately taken out and cooked to 165*f degrees. Corrective Action: Corrected at time of inspection. Predefined Comment: All raw animal proteins must reach a final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code.

Addl Notes: Observed right side cook line fridge open tops holding shredded cabbage, spring mix, and sliced spam 49-55*f degrees. Per manager items had been on the line since 11am. Corrective Action: Corrected at time of inspection. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed cookline rice paddle stored in stagnant water room temperature. Corrective Action: Corrected at time of inspection. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed 3-comp sink drain elbow twisted, while in- use 3-comp sink drains improperly partially onto kitchen floor. Corrective Action: Correct prior to next routine inspection. Predefined Comment: All plumbing must be maintained in good repair at all times.

Addl Notes: Upon starting inspection, inspector observed standing water in the back of house area with the 3 -comp sink. Water was squegged into 3-comp floor drain by manager. Sewage water could be seen inside center room grated drain but was not continuing to back up at time of inspection. Corrective Action: Corrected at time of inspection. Sewage must not back up into establishment. All wastewater must be dumped down a sanitary drain.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
