Addl Notes: Observed numerous cases of raw eggs being stored on shelf above butter and cheese in walk-in cooler. Ready to eat items were moved to safe location at time. Addl Notes: Deli slicers had heavy buildup of food debris under blade guards during inspection. Debris was caked and dry, person in charge was not sure when these areas had been cleaned last and cleaned them at time. Please ensure food contact surfaces are cleaned and sanitized at least every 4 hours while in use and are maintained clean to sight and touch when not in use. Corrective Action: Corrected at time of inspection.

Addl Notes: Chicken broth in walk-in cooler measured 51-52 degrees F, items had been prepared the day before and were stored in walk-in since then. Cooler was not functioning properly and broth measured higher than other food items so it was determined to be an issue with improper cooling. Broth was discarded at time. Please ensure all time/temperature controlled for safety (TCS) food items are cooled from 135 to 70 degrees F in no more than 2 hours, and down to 41 degrees F in no more than 6 hours total. Corrective Action: Corrected at time of inspection.

Addl Notes: Cooked mushrooms, cooked beef, and cooked corn in steam table measured 71 degrees F, it was determined that the steam table was not working. Items had been stocked 3 hours prior and were discarded at time. Please ensure all time/temperature controlled for safety food items are maintained at 135 degrees F or above when hot holding. Corrective Action: Corrected at time of inspection.

Addl Notes: Cooked and raw pork, cooked and raw eggs, butter, and chicken fat in walk-in cooler measured 46-48 degrees F internally. Person in charge said that they noticed the temp rising less than 2 hours earlier and placed service call then. All TCS food items were moved to working coolers at time. Please ensure all TCS food items are maintained at 41 degrees F or below when cold holding. Corrective Action: Corrected at time of inspection.

HACCP plan stipulates that all critical control points must be monitored and documented during process, but no logs were available for review during inspection. Please ensure HACCP procedures are reviewed and conformed with, critical control point documentation must be maintained and available on-site for review for at least 180 days. Corrective Action: Correct prior to re-inspection.

Addl Notes: Establishment was using a large stone grain grinder, which is not NSF approved, to make flour for noodles during inspection. Owner had stated that this equipment was for aesthetics only but it was being used to grind flour during inspection. Person in charge agreed to discontinue using this equipment and will switch to an approved unit as soon as possible. Please ensure all food contact equipment is made with smooth, cleanable materials and is NSF approved for food establishments. Corrective Action: Corrected at time of inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
