2-102.11(A-B) and (C)(1), (4)-(16) - Priority Foundation: Demonstration; Demonstration of Knowledge Addl Notes: Food establishment person in charge (PIC) was unable to demonstrate knowledge of food safety during the inspection. Establishment could not provide a valid CFM certificate during the visit; A priority violation was documented regarding logs for the F'REAL blender machine and the person in charge was unable to answer questions about cleaning and sanitizing dishes, cleaning process for variance approved blender machine, time as control procedures for hot TCS food items, or vomit and diarrheal event clean-up procedures. Establishment PIC must demonstrate knowledge of food safety rules, policies, and procedures. Predefined Comment: A person-in-charge is required onsite at all times, must demonstrate adequate knowledge, and perform all duties as required.

Corrective Action: Correct prior to next routine inspection. Predefined Comment: A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation.

Addl Notes: Discussed vomit and diarrheal event cleanup and procedures with person i charge(PIC). Person in charge did not demonstrate knowledge of clean-up procedures and could not locate or present written procedures at time of inspection. Discussed with PIC the importance of being prepared for vomit and diarrheal events in order to prevent outbreak of illness. Maricopa county vomit and diarrheal procedures provided alongside report for use by the establishment. Corrective Action: Corrected at time of inspection. Predefined Comment: Written procedures for clean-up of vomiting and diarrheal events is required onsite at all times.

2-401.11 - Core: Eating, Drinking, or Using Tobacco Addl Notes: Observed open Styrofoam cup of liquid, salad, and backpack stored on top of canned beverages in the walk-in cooler. Employee claimed the items as their personal belongings and immediately moved the belongings into the designated personal item storage. Corrective Action: Corrected at time of inspection. Predefined Comment: PIC moved employee consumables to the designated area and will retrain on proper eating, drinking, and tobacco use.

Addl Notes: Observed two handwashing sinks on the eastern wall of the building, next to the Ready to Eat (RTE) and self-serve food without disposable hand towels for drying hands. PIC was notified and paper towels were supplied at the time of the inspection. Corrective Action: Corrected at time of inspection. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Predefined Comment: PIC provided hand drying provision for handwash sink at time of inspection.

Addl Notes: Observed no hand soap in the men's restroom at the time of inspection. PIC was notified and hand soap was immediately supplied. Corrective Action: Corrected at time of inspection. Predefined Comment: PIC provided soap for handwash sink at time of inspection.

Addl Notes: Observed cooked pizza in a hot-holding case at 101 degrees F. According to employees, Pizza is removed from the hot case every 4 hours. Sticker, timer, or other marker to indicate how long the pizza had been in the unit was not observed. per cook employee, the time as a control (TAC) sticker was not placed out with the pizza and was discarded. Cook employee was able to provide the sticker indicating 4-hour discard time, at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed F'real machine with manual cleaning record logs adjacent to the machine. ON 4/23/2025, the last entry for the weekly cleaning log recorded was on 04/10/2025. The last marked entry for the daily cleaning log was 3/3/2025. PIC was unaware of the last time that the machine was cleaned and went through its sanitization process, and did not know how to operate the machine. According the approved variance, Logs must be maintained that verify that the F’REAL blender undergoes automatic cleaning and sanitization using the submitted and approved process once every 24hrs and a manually cleaning and sanitizing using the submitted and approved weekly cleaning procedures once every 7 days. PIC was directed to clean immediately and maintain logs here forward.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
