5-202.12 (A) - Priority Foundation: Handwashing Sinks, Installation. Proper temperature, Mixing Faucet. Addl Notes: No hot water available at the handwashing sink. The hot water faucet is in disrepair. When the hot water faucet is turned no hot water is dispensed. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Repair handwashing sink so it dispenses hot water prior to re-inspection within ~10 days. Corrective Action: Correct prior to re-inspection.

Addl Notes: Observed a large chef knife stored on the magnetic knife holder with visible food debris on the blade. Observed visible food debris on electric desktop meat slicer blades. After discussion with the person in charge it was determined the knife and meat slicer were not used today. Please ensure equipment and utensils are cleaned, sanitized, and maintained clean to sight and touch between uses. The person in charge washed and sanitized the knives and meat slicer blades at time of inspection. Corrective Action: Corrected at time of inspection

Addl Notes: Observed a container of coconut milk soup base/66°F in a small pot in the bottom section of the prep refrigerator. After discussion with the person in charge it was determined the soup was cooked at 11AM (4+ hours ago) and had been cooling since. Predefined Comment: Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. Predefined Comment: All TCS foods improperly cooled were discarded by PIC at time of inspection. Discussed cooling time parameters and methods with the person in charge. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a container of Mussman curry paste/80°F (contains cooked garlic) and green curry paste/77°F (contains shrimp paste) held on a cart at room temperature. According to the person in charge the curry pastes were held at room temperature for 2 hours. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Discussed utilizing 4 hour time as a public health control for TCS foods held at room temperature. Corrective Action: Corrected at time of inspection. Repeat Violation: 3rd.

Addl Notes: Observed a container of cooked red curry in the walk in refrigerator with no date mark. After discussion with the person in charge it was determined the red curry was cooked on Sunday (11/2). Predefined Comment: RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Predefined Comment: All RTE/TCS foods were properly date marked at the time of inspection. Discussed establishment date marking policy and importance with the person in charge. Corrective Action: Corrected at time of inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed rice paddle and other in use utensils stored in standing water near the rice cooker. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection. Repeat Violation: 2nd.

PRIORITY VIOLATION-5-202.11 (A) - Priority: Approved System and Cleanable Fixtures; According to Law

Addl Notes: At mop sink observed a Y-valve attached to mop sink faucet equip with an AVB. The installation of a Y-valve on a faucet with or without an AVB is prohibited and is a violation, because the system is not designed, constructed and installed according to law. Person in charge removed the Y-valve at time of inspection. Corrective Action: Corrected at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
