Addl Notes: Corrective Action: Corrected at time of inspection. Noted numerous employees put on gloves and start working without first wash hands in the kitchen. Reviewed when to wash with manager and instructed manager to enforce hand washing.

Back door does not have self-closer and the dumpster is within 30 feet of the backdoor. Provide air curtain or screen door, self-closer, threshold and door sweep for the back door. If air curtain is provided it must be operated by micro-switches.

Addl Notes: Corrective Action: Correct prior to re-inspection. Noted numerous grocery t-shirt bags are used to store foods inside the freezer and non-food grade containers are used to store dry goods. Please use food grade containers and storage bags. Instructed employee to obtain approved materials.

Addl Notes: Corrective Action: Correct prior to re-inspection. Noted a custom-made grinder that does not meet the NSF requirements for food equipment. Owner will remove the equipment off the premises and order commercial equipment if needed. Also, numerous small wares (blenders, food processor, etc.) that are not commercial grade.

Noted hot water at the bar's 3-comp sink is not presence and the hot water at the hand washing sink inside the restrooms are only on partially. Hot water must be at 120*F at all sinks within 45 seconds upon activation. Tempered water at hand sink must be at least 100*F. Provide re-circulation pump and hot water booster to maintain hot water throughout the facility.

5-203.13 - Core: Service Sink Addl Notes: Corrective Action: Correct prior to re-inspection. There is no mop sink available in the facility. Mop water was thrown out the back door. Mop sink must have hot and cold water, backsplash must be FRP, stainless steel or tiles.

Addl Notes: Corrective Action: Correct prior to re-inspection. Noted some of the drainpipes at the bar and the ware washing area are not provided proper air gap. Provide at least 1" air gap from the finished floor for floor sink, hub drain or riser. Provided sample picture of riser and hub drain.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Corrective Action: Correct prior to next routine inspection. Noted the hot and cold water at the bar is backward. Hot water must be on the left side of the faucet and cold on the right side. Noted open un-used water lines are not cap off. Please cap off any un-used water lines.

PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines

Addl Notes: Corrective Action: Correct prior to re-inspection. Noted an un-used/ open water hose is laying inside the floor sink's basin. Water lines cannot be below the drainpipe or inside floor sink. Instructed manager to remove the water line and plug the un-use connection. The hose must be removed.

All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection.

6-501.16 - Core: Drying Mops Addl Notes: Corrective Action: Correct prior to next routine inspection. Noted mops are drying outside of the restaurant. Provide mop hooks to hang them above the mop sink to air dry.

6-201.13 - Core: Floor and Wall Junctures, Coved, and Enclosed or Sealed Addl Notes: Corrective Action: Correct prior to next routine inspection. Noted cove base is missing at cabinet base/ floor junctures, walls and floor junctures. Provide 3/8" radius cove base at above mentioned areas. Also, there are holes on walls, inside the walk-in units, gaps at top of walk-in to the ceiling, around pipe penetrations, etc. Cauk and seal any gaps, cracks and penetrations that has the thickness of a business card. Inside the walk-in units, please use wide range temperature silicone (silver color) and provide missing plugs where missing.

6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Corrective Action: Correct prior to next routine inspection. Noted new dry walls around the ware washing area are not painted. Please use several coats of semi-gloss paint. Also, substrate of the countertop at the bar are not painted. Provide several coats of semi-gloss paint. Noted the floor in the kitchen are damaged, missing, cracks, chipped, etc. Repair/ replace or refinish. Provide washable ceiling tiles inside the bar and above the self-service station. Provide side and top protections for the customer's self-service station.

6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Corrective Action: Correct prior to next routine inspection. Noted there are a lot of items, tools, equipment, containers, etc. that need to be removed/ replaced or not used. Please remove these items off the premise.

Comments/Details

OC re-inspection conducted with the person in charge, Nikpreet Singh. Most items noted in the last report had been addressed. The remodel permit for the new buffet service area is still open to address all remodel areas. Permit is approved.
No County legal action will result from this inspection.
You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) "No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code." County legal action may result due to violations observed during this inspection. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
