The sink near the dish machine was designated as the handwashing sink at time of inspection. Predefined Comment: PIC relocated all items blocking handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed bar dish machine washing glassware with final rinse chlorine concentration of 0PPM. Chlorine sanitizer must measure be between 50-100 PPM. Instructed PIC to discontinue sanitizing food contact surfaces in the bar dishmachine until it is repaired, and sanitizer tests at 50-100 PPM. Predefined Comment: PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed tomato sauce/125°F holding countertop steam well near the pizza station. Observed roasted onions/81°F, cooked trumpet mushrooms/ 81°F , roasted romoscuto/79°F , and gravy/87°F in small metal containers on the side of the brick oven. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. After discussion with the person in charge it was determined all TCS foods were cooked less than 2 hours prior. Predefined Comment: All TCS foods improperly hot held were reheated to 165°F at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed caramelized onion focaccia/69°F in the kitchen on racks. On the counter near the expo station observed whipped ricotta and plated ricotta/70°F. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. After discussion with the person in charge it was determined all TCS foods were removed from cold holding about an hour ago. Instructed person in charge to return all TCS foods to cold holding. Discussed utilizing 4 hour Time as a public health control for TCS foods held at room temperature. Provided guidance document. Corrective Action: Corrected at time of inspection.

Addl Notes: On the dinner menu and beverage menu observed no disclosures in the form of asterisks for raw or undercooked animal foods such as oysters, Hamachi crudo, ossetra caviar, assorted beef steaks, salmon, and Wild Wood Cocktail (raw unpasteurized shelled egg). Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Updated menu with asterisks for all raw or undercooked animal foods at time of inspection with the person in charge. Reminder statement present on menu. Corrective Action: Corrected at time of inspection.

Addl Notes: In the upstairs freezer and meat walk in cooler observed raw sea bass (10/3 date), raw salmon (9/19 date), raw chicken (9/9 date), crab (10/4) in ROP packaging. Product is packaged onsite. Predefined Comment: A variance/HACCP must be obtained prior to conducting any specialized processes. Variance/HACCP applications are available through the customer portal "Permit Center" at https://maricopacountyaz energovweb.tylerhost.net/apps/selfservice#/home. The establishment will no longer ROP package fish and will only ROP package chicken and steaks for less than 48 hours with date and time. Except for fish that is frozen before, during, and after packaging and bears a label indicating that it is to be kept frozen until the time of use, a food establishment must not package fish using a reduced oxygen packaging method per 3-502.12. ROP of raw animal food for over 48 hours requires an approved HACCP plan. PIC removed ROP product from package at time of inspection. Corrective Action: Corrected at time of inspection.

Predefined Comment: Sanitizing using high temperature warewashing machines requires an irreversible measuring device to ensure food contact surfaces reach 160°F. Provide approved irreversible measuring device prior to re-inspection. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
