Addl Notes: At front make table, in hot holding well, cooked beef with internal temperatures of 120*F. Per employee, beef was reheated on cook top and placed in hot holding well and ready for service. Employee reheated cooked beef on cook top to 165*F. Maintain TCS (time/ temperature control for food safety) foods at a hot holding temperature of 135*F or higher at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: In walk in cooler, TCS foods (tomato based salsas, cooked chicken, cooked beef, raw beef, raw chicken) with internal temperatures of 44*F. Per employee, TCS foods were at 41*F at time of opening/ within 4 hours. Instructed employee to provide ice baths to tomato based salsas, cooked chicken, cooked beef, and move raw beef and raw chicken to prep table cooler to rapidly cool back down to proper temperature. Maintain TCS (time/ temperature control for food safety) foods at a cold holding temperature of 41*F or less at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: Walk in cooler with an ambient air temperature of 46.5*F, taken with inspector min/max irreversible thermometer. Discontinue storing of TCS foods in walk in cooler until repaired. Repair company was called at time of inspection. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below at all times. Repair cold holding equipment such that it maintains TCS foods at 41°F or below. Large prep table cooler and tall reach in refrigerator are holding TCS foods and ambient temperatures <41*F. Corrective Action: Correct within 24 hours.

3-501.15 (B) - Core: Cooling Methods; Covered and Properly Arranged Containers. Addl Notes: In tall single door glass front refrigerator, cut lettuce/ 75*F and cooked rice/ 55*F prepared and cooling in shallow covered containers. Per employee, lettuce and rice were prepared within normal time parameters. Cut lettuce and containers of cooked rice were uncovered to continue the cooling process. Discussed proper cooling methods with employees. (B) When placed in cooling or cold holding EQUIPMENT, FOOD containers in which FOOD is being cooled shall be: (1) Arranged in the EQUIPMENT to provide maximum heat transfer through the container walls; and (2) Loosely covered, or uncovered if protected from overhead contamination, as specified under Subparagraph 3-305.11(A)(2), during the cooling period to facilitate heat transfer from the surface of the FOOD. 5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Second Consecutive Violation. Per Plan Review OC Inspection, Sprayer is currently right at the edge of the flood rim if left hanging loosely. Adjust/replace sprayer at 3-compartment sink so it maintains a minimum 1" air gap above flood rim of sink and reattach to wall. At soda fountain, drain pan not draining properly. Repair to allow water to drain properly. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
