Addl Notes: Observed no written procedures for clean-up of bodily fluids, vomiting, and diarrheal events on site or spill biohazard kit. Written procedure for clean-up of vomiting and diarrheal events is required onsite at all times. Helped the manager to purchase spill biohazard kit at time of inspection and educated manager with procedures and documentation for bodily fluid clean-up procedures. This is a repeat violation. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a fan was put inside the handwash sink in the kitchen and two buckets were placed in front of the sink. The handwash sink was not accessible at time of inspection. All handwash sink must be accessible at all times. Manager removed the fan and the buckets at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed reach-in fridge by the 3-compartment sink have raw eggs stored above RTE cheese and chashu. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. Manager moved the raw eggs to the bottom shelf at time of inspection. Corrective Action: Corrected at time of inspection. This is a repeat violation.

Addl Notes: Observed chlorine concentration at 3-compartment sink at bar area measured less than 10 ppm at time of inspection. Reviewed the sanitizer concentration parameters with manager. Manager re-prepared the chlorine sanitizer to 50-100 ppm at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed meat slicer stored as clean with food debris heavily accumulated. Discussed utensil and equipment cleaning frequency with manager. Manager dissembled the meat slicer and put it at ware washing station for cleaning and sanitizing at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed tofu, cooked eggs, cooked pork, cooked chicken in the coolers and fridges without datemarks at time of inspection. Manager stated the above food items were prepared 48 hours prior to inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Manager put proper datemarks on above food at time of inspection. Corrective Action: Corrected at time of inspection. This is a repeat violation.

6-501.111 (A-B, D) - Core: Controlling Pests; Monitoring Addl Notes: Observed dead cockroaches under ware wash machine and wok station. Manager stated they get monthly pest control services. Advised manager to remove and discard of dead pests frequently when found. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem. Corrective Action: Correct prior to next routine inspection. This is a repeat violation.

Observed food debris and black organic under ware wash station. Conduct a deep clean of the walls and floors throughout the kitchen. Corrective Action: Correct prior to next routine inspection. This is repeat violation.

4-202.18 - Core: Ventilation Hood Systems, Filters Addl Notes: Observed ventilation hood systems with accumulation of grease. Filters or other grease extracting equipment shall be designed to be readily removable for cleaning and replacement if not designed to be cleaned in place. Advised manager to frequently clean ventilation hood systems and filters to ensure safety and prevention of grease fires. Corrective Action: Correct prior to next routine inspection. This is repeat violation.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
