Addl Notes: Observed handwashing sink in the front of house with a coconut stored inside the sink basin at time of inspection. Observed handwashing sink in the back of house filled with dishes and there was a cart stored in front of the hand sink at time of inspection. Observed no paper towels at the handwashing sinks in the facility at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided hand drying provision for handwash sink at time of inspection. PIC will ensure establishment discontinues use of handwash sinks for all activities that are not for handwashing. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed raw chicken throughout the walk-in cooler stored above curry sauces, yogurt, and cooked tofu at time of inspection. Observed raw fish stored above yogurt at time of inspection. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. Person in charge (PIC) properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Emailed PIC with separation of storage guidance document at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed multiple pans of cooked chicken sitting out at ambient temperature with temperatures between 95 to 103*F at time of inspection. Per cook, the chicken was cooked 30 minutes to an hour prior to inspection. Cook moved the cooked chicken to walk-in cooler to rapid cool at time of inspection. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed water bottles being stored in ice machine where ice is being utilized for customers drinks at time of inspection. Ice used as an exterior coolant may not be used as an ingredient in any food/drinks. Person in charge (PIC) removed bottles from ice wells, burned the ice, sanitized the well, and refilled the ice at time of inspection. Corrective Action: Corrected at time of inspection.

3-305.11 - Core: Food Storage - Preventing Contamination from the Premises Addl Notes: Observed containers of cooked chicken stored on the floor and under a prep table in the cooking area at time of inspection. Observed multiple food storage containers stored directly on the floor at time of inspection. Food and food containers must be stored at least 6 inches off the ground at all times to prevent contamination from the premises. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed unusable quat test strips in facility at time of inspection. Establishment utilizes quat sanitizer for sanitizing food contact surfaces at time of inspection. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection on 4/30/2026.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
