Comments:
PRIORITY VIOLATION-3-302.11 (A1-2) - Priority: Packaged and Unpackaged Food-Separation, Packaging, and Segregation; Protection from Cross Contamination. Addl Notes: Observed a piece of raw beef on the surface of the shredded lettuce in the prep table. Per PIC the lettuce is used as a garnish where cooked short ribs are placed on top and served. Discussed with PIC All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Embargoed the Lettuce and PIC discarded at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed knives with food debris stored on the magnetic strip that also has clean knives. Per PIC the magnetic strip is for clean knife storage. Discussed with PIC All food-contact surfaces must be cleaned to sight and touch. Food-contact surfaces must be properly sanitized after washing and rinsing. The knives were removed from the strip and cleaned and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed in the prep table in the kitchen with cooked noodles in a bag above the refrigerator line at 48*F and raw white tuna at 50*F with raw salmon at 48*F in the prep table on the sushi prep line above the refrigerator line measured with a probing thermometer. Per PIC the items were in the unit for more than 4 hours. All TCS foods improperly cold held were embargoed and discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed 2 large containers of white rice cooked with a time stamp of 11- 3. Per PIC there is no policy and they discard by the time on the stamp. At the time of this the inspection it was 3:36. Discussed with PIC Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Items shall be discarded after the 4 hour window. Embargoed the cooked white rice and PIC discarded at time of inspection. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage
Addl Notes: Observed butane torches stored facing cutting boards and clean dishes. Also observed a cough medicine stored above the prep table in the kitchen. Discussed with PIC medicines and chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals and medicine were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Description: Proper Cooling Methods Used; Adequate Equipment for Temperature Control
Comments:
3-501.15 (B) - Core: Cooling Methods; Covered and Properly Arranged Containers. Addl Notes: Observed in a working prep table to have cooked chicken, cooked beef, fish cakes, gyoza, carrot and peas mix at internal temperature of 44*F to 48*F measured with a probing thermometer. Per PIC the cooked meats were cooked and placed in the unit less than 4 hours to inspection. Fish cakes, gyoza, carrots and peas temperature measured above 41 due to hot items being placed in the cooler as discussed with the PIC. Discussed with PIC TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Discussed with PIC a prep table is a cold holding table and not adequately designed to cool foods and can affect proper holding temperature within the unit. PIC relocated foods to the walk in to rapidly cool. Corrective Action: Corrected at time of inspection.

Of Note: Discussed with PIC 1) Keep employee drinks and food low and store in a way that prevents contamination of food, food contacts surfaces, clean linens, and equipment. 2) allow dishes to properly airdry before wet stacking. 3) single use containers are not designed to be washed and reused for food, sauces or dry ingredients.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
