Addl Notes: In the handwashing sink on the cook line observed a large Cambro of bread stored in the basin of the handwashing sink. At the bar observed both bar handwashing sinks set up as dump sinks. Observed food debris from dumped drinks in the sink. Facilities must be maintained in a condition that promotes handwashing. Handwashing sinks may only be used for handwashing. Predefined Comment: PIC relocated all items blocking handwash sink at time of inspection. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. Corrective Action: Corrected at time of inspection.

A written agreement or statement from the supplier stipulating that the fish are frozen to a temperature for specified time must be retained on site of the operation or a written agreement or statement from the supplier or aquaculturist stipulating that the fish were raised and fed as specified in subparagraph 3-402.11(b)(4) shall be obtained by the person in charge and retained in the records of the food establishment. Obtain prior to re-inspection on 6-12-2026. Document may be sent to inspector electronically. Corrective Action: Corrected at time of inspection.

Addl Notes: In the walk in refrigerator observed a hotel pan of raw chicken skewers stored on a shelf directly above raw clams. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. In the drawer refrigerators below the grill observed a container of raw chicken stored directly above a container of raw shrimp. No overhead contamination observed. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Provided guidance document. Corrective Action: Corrected at time of inspection. Repeat Violation: 2nd.

Addl Notes: Observed chorizo butter/72°F, hummus/63°F, tomato oil/73°F, house made ranch/ 72°F, and house made aioli/73°F held at room temperature on the counter across from the grill. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. According to the PIC all TCS foods had been out on the counter for 1-2 hours. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Observed milk/50°F and heavy cream/51°F held in sliding door display refrigerator. Ambient temperature of bar refrigerator measured at 49.8°F with inspector's min/max thermometer. Discontinue use of refrigerator for storing TCS foods until it maintains 41°F or less. Milk and heavy cream were discarded. Corrective Action: Corrected at time of inspection.

6-501.115 - Priority Foundation: Prohibiting Animals Addl Notes: Observed a customer in the establishment with a parrot on the shoulder. Live animals prohibited on premises of food establishment, with the exception of fish in aquariums, shellfish, patrol dogs (in certain areas), and service animals. Guest left voluntarily at time of inspection. Reviewed what animals are allowed in food establishments. Emailed ADA service animals guidance document. Corrective Action: Corrected at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
