Addl Notes: Observed no fecal-vomit kit or procedure on the premises of the establishment. Person in Charge (PIC) stated they do not have a kit or a written procedure. Provided PIC Vomit and Diarrhea Clean up Guidance on site and PIC located a bottle of chlorine bleach and properly stored it under the three compartment sink. Written procedures for clean-up of vomiting and diarrheal events is required onsite at all times. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed no handwashing sink signage at any of the handwashing sinks located in the establishment. Advised Person in Charge (PIC) to create handwashing signs and install them at or near the sinks in the food prep area and restroom prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Comments:
7-202.11 - Priority Foundation: Restriction-Presence and Use Addl Notes: Observed a can of Raid fly and bug pest remover stored on the bottom shelf of the dry storage unit. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. Advised PIC to remove the Raid spray and PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.
PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage
Addl Notes: Observed a can of degreaser stored above in use soda bibs. Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

6-202.15 - Core: Outer Openings, Protected Addl Notes: Observed front door with padding taped to left side preventing the right side from being able to tightly shut and form a seal for proper protection against pests. Informed PIC that the establishment must be maintained free of pests and the door must be fixed as soon as able. Correct prior to next routine inspection.

4-901.11 - Core: Equipment and Utensils, Air-Drying Required Addl Notes: Observed blender pitchers stacked together while still wet, preventing the equipment from properly air drying. All equipment and food contact surfaces must be air-dried after properly cleaning and sanitizing. Corrective Action: Correct prior to next routine inspection.

Comments:
6-403.11 - Core: Designated Areas-Employee Accommodations for Eating/Drinking/Smoking Addl Notes: Observed employee's lunchbox and water bottle being stored on a shelf in the dry storage area above containers of doughnut filling, an employee's lip balm and uncovered water cup were stored above a prep station not currently in use, and an employee's can of soup and takeout box were stored in the walk in above dough and milk. Informed PIC that employees must have a designated area for employees to eat, drink, and use tobacco and needs to be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
