Addl Notes: Observed MF Unit offer for sale home cooked TCS desserts and frozen drink mixes. TCS desserts included tres leches and flan, TCS frozen drinks included dairy milk based agua frescas mixes. Predefined Comment: All food must be obtained from an approved source as defined by law. Predefined Comment: All food from unapproved source was discarded at time of inspection. The person in charge removed all home cooked food items from sale at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed employee washing cutting board then placing cutting board in sanitizer solution with less than 10ppm chlorine then placing cutting board inside the handwash sink to air dry. The person in charge made new sanitizer solution with 50-100ppm chlorine and properly washed, rinsed, sanitized the cutting board. Predefined Comment: Food-contact surfaces must be properly sanitized after washing and rinsing. Predefined Comment: Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Predefined Comment: Food-contact surfaces sanitized using chlorine sanitizer must be sanitized with a contact time of least 10 seconds. Predefined Comment: PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked shrimp and cooked steak with internal temperatures between 109-120F in the steam wells by the cook top. The person in charge stated the food items had been placed for hot holding less than 2 hours ago. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Predefined Comment: All TCS foods improperly hot held were reheated to 165°F at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed TCS food items with internal temperature between 47-69F in the reach in cooler. TCS food items included raw shrimp, raw fish, dairy butter, cut cabbage, and shredded mozzarella cheese. The person in charge stated the food items had been placed for cold holding 5 hours ago. Predefined Comment: All TCS foods improperly cold held were discarded by PIC at time of inspection. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

All medicines were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed reach in cooler with minimum ambient temperature of 50F as measured by inspector min/max thermometer. MF Unit has no other cold holding equipment. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection.

Description: MCEHC Mobile Food Requirements

MCEHC Chap. 8, Sec. 3, Reg. 5f - Sanitary toilet facilities for use by employees shall be readily available within 200 feet of all mobile food establishments that are operated at the same site for more than a one-hour period. Addl Notes: Observed no sanitary toilet use agreement. MF Unit is using a chemical toilet for employee use. The Maricopa County Environmental Health Code requires that all mobile food establishments operating at the same site for more than a one-hour period provide sanitary toilet facilities for use by employees within 200 feet of the operation. A toilet use agreement should be provided prior to permit approval and the approved sanitary toilet facilities must meet all of the criteria described below. Please note that chemical toilets do not meet these requirements. Corrective Action: Correct at once.
MCEHC Chap. 8, Sec. 3, Reg. 5e - Mobile food establishments serving time/temperature control for safety foods shall maintain a log of visits to the approved commissary or food service establishment as required by this Department. This log shall be maintained using a Department approved format and maintained with the establishment at all times. The logs shall be maintained for a period of 30 days and should be available for review by the Department. Addl Notes: Observed MF Unit unable to provide current commissary visit logs. Observed most recent commissary log entry from February 2026 more than 5 months ago. Operator must maintain and provide daily logs of commissary visits. Corrective Action: Correct at once.
MCEHC Chap. 8, Sec. 3, Reg. 9e - MF Type III-All cooking, processing, preparing, grilling, assembly, storage, and service of any food or beverage shall be conducted inside the permitted vehicle. Addl Notes: Observed limes next to a cutting board and cutting utensils on a table outside the MF Unit. Observed no hand wash station or protection of utensils and food items from contamination. Discussed food may only be prepared inside the permitted MF Unit. The person in charge moved all food and utensils inside the MF Unit at the time of inspection. Corrective Action: Corrected at time of inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
