Comments:
PRIORITY VIOLATION-3-302.11 (A1-2) - Priority: Packaged and Unpackaged Food-Separation, Packaging, and Segregation; Protection from Cross Contamination. Addl Notes: Observed ceviche with raw shrimp in a metal pan with no lid stored above RTE salsa container. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code, ceviche moved to bottom shelf and salsa moved to shelf above to satisfy requirements. Corrective Action: Corrected at time of inspection, PIC rearranged items.

Addl Notes: Observed raw, sliced tomato in top service area of cold holding unit (between hand sink and hot holding unit) reading 56°F, along with shredded cheese reading at 52°F, and shell-on eggs at 51.9°F inside bottom storage area of unit. Per PIC, all foods had been placed in unit around 8am this morning. PIC long-stem thermometer read 40°F, but inspector min/max thermometer read 47.1°F after at least 10 minutes. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. After attempts to lower temperature with ice bath, foods were not exhibiting signs of cooling and surpassed the 4 hour time limit for out of temperature foods, therefore requiring embargo of the following items: shredded lettuce, minced cabbage, shell-on eggs, raw chorizo, ham slices, cooked steak, sliced tomato, and shredded cheese. Once removed, a min/max thermometer was once again placed in the cold holding unit and read at 43°F, as well as PIC voluntarily hiring HVAC worker to check unit. Corrective Action: Corrected at time of inspection, food voluntarily embargoed, unit brought down to temperature.

Addl Notes: Observed cold holding unit with flip-down covers reading 47.1°F and unable to lower temperature due to consistent use and opening of top service portion. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Upon removal of out of temperature foods, the cold holding unit came down to 43.1°F to satisfy the needs of the unit. Corrective Action: Corrected at time of inspection.

6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed what appeared to be small sewar roaches moving between holes of anti-slip mats on floor of unit. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Per PIC, an insect control specialist was on premises at 5am the morning of the inspection. PIC is advised to increase frequency of pest control application until pests are controlled. Corrective Action: Correct prior to re-inspection by 08/01/2026.

Description: MCEHC Mobile Food Requirements
Comments:
MCEHC Chap. 8, Sec. 3, Reg. 5e - Mobile food establishments serving time/temperature control for safety foods shall maintain a log of visits to the approved commissary or food service establishment as required by this Department. This log shall be maintained using a Department approved format and maintained with the establishment at all times. The logs shall be maintained for a period of 30 days and should be available for review by the Department. Addl Notes: Observed no Commissary Log available upon request. Any time mobile units visit their commissary, the commissary log must be filled out and stored on the unit to support requirements of mobile food code. PIC was informed of requirement and advised to carry Commissary Log at all times. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
