5-202.12 (A) - Priority Foundation: Handwashing Sinks, Installation. Proper temperature, Mixing Faucet. Addl Notes: Observed no hot water under pressure at handwash station near Southwest end of the facility. Water shall measure 100°F through a mixing valve or combination faucet. Person in charge obtained hot water at time of inspection.

Addl Notes: Hand soap dispenser at handwash station in Southeast section of the facility did not dispense hand soap. Hand soap must be available to properly wash hands. Addl Notes: Observed opened, prepped containers on burger spread sauce, tarter sauce, cut tomatoes, and garlic in oil stored in walk-in refrigeration unit located in the Southwestern region of the facility with temperatures ranging from 34*F to 40*F with dates labeled 3/27, 8/6, 7/2. Person in charge stated these were the dates the items and been prepped. Also observed containers of cooked macaroni and cooked steak stored in walk-in refrigeration unit located in the Northern section of the facility without date markings. It is unknown how long food items had been in refrigeration unit. A food shall be discarded if: o It exceeds the 7 days at 41ᵒF, except time it is frozen o It is in a container or package that does not bear a date/day and has been held for > 24 hours and facility has no datemarking system in place. Food items were embargoed at time of inspection.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed small leak at faucet of three-compartment sink located in Northern section of facility. Also observed hot water knob at hand wash station located in Northern section of facility in disrepair. In addition, a small leak was observed at faucet of mop sink located outside the facility on the East side of the facility. All plumbing shall be maintained and in good repair. Please correct prior to next routine inspection.

Comments:

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed standing water on the floor inside walk-in refrigerator located in Northern section of facility. Person in charge was not aware of the presence of the standing water. Also observed red and orange organic matter build up on boxes, storage racks, and floor on walk-in refrigeration unit located in the Southeastern section of the facility. Person in charge cleaned standing water and build up at time of inspection.

6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Observed multiple miscellaneous items, refuse, and equipment scattered along the outside of premises. Person in charge stated the items were to be thrown out or repaired. When asked how often bulk trash had been thrown out, the person in charge stated it has not been taken care of in the past 6 months. This violation refers to the removal of litter and articles that are not needed for the operation or maintenance of the establishment. • Old equipment that is no longer used is being kept in the establishment. • Rubbish not removed from the establishment. Please correct prior to next routine inspection.

6-303.11 - Core: Intensity-Lighting Addl Notes: Observed 2 overhead light fixtures located in women's restroom with insufficient light intensity. The light intensity shall be at least 108 lux (10 foot candles) at a distance of 75 cm (30 inches) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning; • At least 215 lux (20 foot candles): o At a surface where food is provided for consumer self-service such as buffets and salad bars or where fresh produce or packaged foods are sold or offered for consumption, o Inside equipment such as reach-in and under-counter refrigerators; and o At a distance of 75 cm (30 inches) above the floor in areas used for handwashing, warewashing, and equipment and utensil storage, and in toilet rooms; and • At least 540 lux (50 foot candles) at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor. Please correct prior to next routine inspeciton.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
