Addl Notes: Observed clothes, dish rags and chemicals blocking the hand sink. Informed PIC that hand sink must be unblocked and available at all times. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. PIC relocated all items blocking handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed hand sink only dispensing water above 120 degrees and employees were not able to wash their hands. Informed PIC that handwash sinks must be supplied with hot water around 100 degrees. Facilities must be maintained in a condition that promotes handwashing. Owner arrived and repaired the sink, so the hot and cold water functioned and employees could effectively wash their hands. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed saluted onions, roasted jalapenos, cooked chicken and cooked beef hot holding at 127-131 degrees. Informed PIC that except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Per PIC food was cooks 30 minutes prior to inspection. PIC instructed employee to return food to the flattop and reheat to 165 degrees. All TCS foods improperly hot held were reheated to 165°F at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed sliced tomatoes, sliced cabbage, cooked salsa, cooked beef, shredded jack cheese, sliced melon and cooked chicken cold holding at 47-66 degrees. Per PIC food was placed in the refrigerator greater than 2 hours prior. Informed PIC that except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed secondary upright refrigerator with an ambient air temperature of 54 degrees. Informed PIC that cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. PIC was able to remove non-tcs foods blocking refrigerator fans turned down the temperature on the units thermometer panel. Refrigerator returned to working order at time of inspection. Corrective Action: Corrected at time of inspection.

6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Observed 3 compartment sink filled with employee scooter, and other boxed. Per PIC 3 compartment sink is being used as storage space due to rewashing happening at commissary (FD-82632). Informed PIC that unit must be cleared of all unused and unnecessary equipment. Corrective Action: Correct prior to next routine inspection.

MCEHC Chap. 8, Sec. 3, Reg. 1(a)(4) - Mobile Food Type III is not operating as classified. Addl Notes: Observed 2 waterboys willed with TCS agues frescas cold holding at 35 degrees. Informed PIC that all cooking, processing, preparing, grilling, assembly, storage, and service of any food or beverage shall be conducted inside the permitted vehicle. PIC moved waterboys inside the unit. Corrective Action: Corrected at time of inspection.
MCEHC Chap. 8, Sec. 3, Reg. 1(a)(4) - Mobile Food Type III is not operating as classified. Addl Notes: Observed cooler in seating area outside the unit holding sliced tomatoes, sliced cabbage and other non TCS. Per PIC this is a self service area for costumers to grab their own toppings. Informed PIC that all food held for consumer self-service shall be wrapped or effectively dispensed to protect from contamination. PIC placed all TCS foods inside mobile unit and removed cooler from seating area. Corrective Action: Corrected at time of inspection.
MCEHC Chap. 8, Sec. 3, Reg. 5e - Mobile food establishments serving time/temperature control for safety foods shall maintain a log of visits to the approved commissary or food service establishment as required by this Department. This log shall be maintained using a Department approved format and maintained with the establishment at all times. The logs shall be maintained for a period of 30 days and should be available for review by the Department. Addl Notes: Observed no commissary logs available at time of inspection. Informed PIC that 30 days worth of commissary logs are to be available for inspectors at all times. Corrective Action: Correct prior to next routine inspection.
MCEHC Chap. 8, Sec. 3, Reg. 5c - Mobile food establishments shall provide a commissary agreement to the Department upon request, at permit issuance or renewal, and as a part of the documents submitted during plan review for a new permit. Ice cream vendors that only sell or dispense individually packaged, commercially processed items are exempt from providing a commissary agreement. Addl Notes: No Commissary agreement on file. Provided Owner with blank copy of commissary agreement at time of inspection. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
