Description: Proper Hot Holding Temperatures
Comments:

Addl Notes: Observed cooked beans(101°F ), cooked rice (83°F), and roasted onions and peppers (99°F ) in hot holding unit reporting below 135°F. Hot holding unit had not been turned on properly. Unit was turned on and food was discarded at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Per PIC, the food was out of temperature for an unknown amount of time. All TCS foods improperly hot held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

3-501.19 (A1, B2, C2-3) - Priority Foundation: Time as a Public Health Control; Documentation. Addl Notes: Observed cooked fried shrimp in container on counter reporting at 75°F. PIC said that the shrimp was fried and then left on counter to cool slightly before being added to cold sushi rolls. PIC confirmed that the shrimp had been there for just under 4 hours, and that they usually discard the food after its been out a while rather than keep it hot. All TCS foods being held under time as a control were properly time stamped at time of inspection, then discarded during the inspection once the time to discard was reached. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Must provide written time as a control procedures prior to re-inspection. Corrective Action: Corrected at time of inspection.

Description: Sewage and Waste Water Properly Disposed

5-402.14 - Priority Foundation: Removing Mobile Food Establishment Wastes Addl Notes: Observed unit wastewater tank dripping wastewater from a small leak underneath the unit near the exit valve. Sewage and other liquid wastes shall be removed from a mobile food establishment at an approved waste servicing area or by a sewage transport vehicle in such a way that a public health hazard or nuisance is not created. Leak was fixed at time of inspection. Corrective Action: Corrected at time of inspection.

Description: MCEHC Mobile Food Requirements
Comments:
MCEHC Chap. 8, Sec. 3, Reg. 8n, 9s - MF Type II and Type III-Wastewater holding tanks shall be emptied into an approved sewage disposal system. Addl Notes: Leak observed at wastewater tank; Operator also confirmed that wastewater was emptied into another receptacle on site that was then emptied off site. Per MCEHC, unit must report to commissary to dump water directly int an approved sewage disposal system. Corrective Action: Correct at once.
MCEHC Chap. 8, Sec. 3, Reg. 6b, 8c, 9b, 10c, 11b - All time/temperature control for safety foods shall be prepared on the same day of sale or service. Time/temperature control for safety foods shall not be held over from a previous day's operation unless otherwise approved by the Department. Addl Notes: PIC confirmed that the TCS items on mobile unit were taken from restaurant freezer and then placed on unit roughly 24 hours prior to inspection. Per MCEHC, all TCS items on mobile unit shall be prepared on same day of service, and shall not be stored int he vehicle overnight. Corrective Action: Correct at once.
MCEHC Chap. 8, Sec. 3, Reg. 5b - Mobile food establishments shall operate from an approved commissary or other food service establishment as required by this Department, and shall report at least daily to such location for supplies, food storage, vehicle and equipment cleaning, waste disposal, and service operations. Addl Notes: Observed current commissary logs and agreement, but unit had flat tires and was immobile at time of inspection. Upon questioning Operator, they confirmed that the unit was not moved, but rather the wastewater tank was drained into another container, which was emptied at the commissary. PER MCEHC, Mobile food establishments shall operate from an approved commissary or other food service establishment as required by this Department, and shall report at least daily to such location. Mobile food units must be mobile, and must move to commissary for service operations. Operator attempted to fix tires at time of inspection. Corrective Action: Correct at once.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
